Prunes and Bone Density: What the Evidence Shows
Randomised trials suggest eating around 50g of prunes a day (5-6 prunes) helps preserve hip bone density in postmenopausal women. Here's the UK evidence, the dose, and honest caveats.
Do prunes actually improve bone density?
In postmenopausal women, the evidence is genuinely encouraging — but the honest finding is that prunes help preserve bone density rather than rebuild it. This is one of the few foods with proper randomised controlled trial evidence behind its bone claims, which sets it apart from most "superfood" headlines.
The landmark study is the 12-month Prune Study, a randomised controlled trial in postmenopausal women published in the American Journal of Clinical Nutrition De Souza et al., American Journal of Clinical Nutrition, 2022. Women who ate prunes daily held on to their hip bone density, while the group who ate no prunes lost it over the year.
That's the key nuance: prunes appear to slow or prevent the bone loss that naturally speeds up after menopause, which matters because the hip is one of the most serious sites for a fragility fracture.
What does the strongest trial actually show?
Eating around 50g of prunes a day preserved total hip bone density over 12 months, while the control group's density fell significantly. In the trial's detailed results, a statistically significant difference in total hip bone density emerged between the control and 50g-prune groups (a group-by-time interaction, p = 0.030) De Souza et al., Current Developments in Nutrition, 2022.
Interestingly, a larger 100g daily dose did not do better. That higher-dose group had a very high drop-out rate (around 41%), largely because eating that many prunes every day is hard to sustain De Souza et al., Current Developments in Nutrition, 2022. That's why researchers settled on the lower 50g dose as the practical, effective one — it worked without being a chore to keep up.
How many prunes, and how much per day?
The evidence-based amount is about 50g a day — roughly 5 to 6 prunes. That is the dose that preserved hip bone density in the Prune Study, and it is a realistic daily habit rather than a large, off-putting portion.
An earlier and equally important 12-month randomised trial used a bigger dose. In 160 postmenopausal women, 100g of dried plum a day (about 10 prunes) significantly improved bone density at the forearm (ulna) and spine compared with a dried-apple control — with both groups also taking calcium and vitamin D Hooshmand et al., British Journal of Nutrition, 2011.
So the trial evidence spans roughly 50-100g a day. Given that the higher dose is harder to stick to and offered no clear extra benefit in the later study, 5-6 prunes a day is a sensible target for most people.
Why might prunes protect bone?
The most likely explanation is that prunes shift the balance of bone remodelling slightly away from breakdown — but the mechanisms are still being worked out and are not fully confirmed in humans.
A comprehensive review in Nutrients points to several plausible routes Wallace, Nutrients, 2017:
- Polyphenols. Prunes are unusually rich in these plant compounds. In animal and cell studies, dried plum extracts appear to enhance bone formation and inhibit bone resorption (breakdown) — though this is laboratory evidence, not proof in people.
- Slower bone turnover. In human data, 100g of prunes a day slowed the overall rate of bone turnover, which can help protect density over time.
- Vitamin K. Prunes contain more vitamin K than most common fruits, a vitamin involved in bone metabolism Wallace, Nutrients, 2017.
What the evidence does not show
Being clear about the limits is what separates honest nutrition advice from marketing. Three caveats matter.
First, almost every trial has been in postmenopausal women — mostly in the US, not the UK. Because that group loses bone rapidly after menopause, they are where a protective food is easiest to detect. There is very little data in men, premenopausal women, or younger people, so the findings should not be assumed to transfer to them.
Second, the effect is preservation, not reversal. Prunes helped women keep bone density they already had; this is not evidence that prunes rebuild a skeleton already weakened by osteoporosis.
Third, this field is frequently funded by the prune industry, and the review above noted that no long-term observational (real-world population) studies had confirmed the trial findings Wallace, Nutrients, 2017. The randomised evidence is real and worth acting on — but it is a modest, food-level benefit, not a cure.
Should prunes replace treatment or screening?
No — prunes are a helpful addition to a bone-friendly diet, not a substitute for medical care. The Royal Osteoporosis Society's guidance is that a healthy, balanced diet across all the food groups is what gives your bones the nutrients they need; single foods work within that pattern, not instead of it Royal Osteoporosis Society, Eating well for your bones, 2024. How would you know if it's working?
The only way to know your bone density is to measure it — and the prune trials themselves show why repeat measurement matters. Those studies detected changes over 6 to 12 months. To see whether your bones are holding steady, you need a baseline reading and a follow-up scan on a similar timescale.
