No — a DEXA (bone density) scan is designed to measure bone density and diagnose osteoporosis, not to detect or rule out cancer. Here's what a bone scan really checks, and which tests investigate suspected cancer.
Will a DEXA scan show cancer?
No. A DEXA (also written DXA) scan is designed to measure your bone density — how strong your bones are — not to detect, confirm or rule out cancer. It is used to diagnose osteoporosis and estimate your risk of breaking a bone, and it is not a cancer screening test.
This is stated plainly by the people who set the UK standard for bone health. The Royal Osteoporosis Society says a bone density scan "is not done to provide information about cancer, pain or osteoarthritis" Royal Osteoporosis Society, 2024. So if you're booked in for a bone scan, or you've had one, it is not a test that either finds or rules out cancer — it answers a different question entirely.
What is a DEXA (bone density) scan actually for?
A DEXA scan measures your bone mineral density (BMD) — the amount of mineral packed into your bones — to tell whether they are healthy, thinning, or already osteoporotic. It uses very low-dose X-rays, usually across the spine and hip, and is a short, painless procedure NHS, 2024.
The result comes back as a T-score, which compares your bone density to that of a healthy young adult:
- Above -1 — normal bone density.
- -1 to -2.5 — low bone density, known as osteopenia.
- -2.5 or below — osteoporosis NHS, 2024.

Why can't a bone density scan detect cancer?
Because a DEXA scan is built to do one narrow thing well — quantify bone mineral — not to image or characterise soft tissue, lumps or tumours. It produces a density measurement, not the kind of detailed anatomical picture a cancer investigation needs. That focus is exactly what makes it accurate for osteoporosis, and exactly why it cannot serve as a cancer test.
There is one nuance worth being honest about. A DXA scan can occasionally reveal an incidental finding — something unexpected in the scanned area that the radiologist notes in the report PubMed (PMID 33040138), 2020. But noting something is not the same as diagnosing it. The scan cannot say what an incidental finding is; if anything looks like it needs a closer look, it is followed up with other tests, not characterised by the bone scan itself.
Which tests actually investigate suspected cancer?
If cancer is suspected, it is investigated with a completely different set of tests — and the right first step is to speak to your GP. Cancer Research UK lists the tools used to investigate cancer as biopsy, CT, MRI, ultrasound, X-ray, PET and PET-CT scans, blood tests and endoscopy Cancer Research UK, 2023. A bone density scan is not on that list, because it does a different job.
If you have noticed a change in how you feel that has lasted a few weeks, it is safer to get it checked. A change usually does not mean cancer — but if it is, the earlier it is found, the more likely treatment is to be successful Macmillan Cancer Support, 2024. Your GP can assess your symptoms and arrange whichever of the tests above are appropriate.
Is DEXA ever used in cancer care?
Yes — but the other way round. In oncology, a DEXA scan is not used to find cancer; it is used to monitor bone thinning that some cancer treatments can cause. NHS DXA services scan oncology patients who are on treatment that can affect their bones Cambridge University Hospitals NHS FT, 2024, and Cancer Research UK explains that a DEXA scan is used for people having, or who have had, cancer treatment that can make bones thinner and weaker Cancer Research UK, 2023.
So if you have been sent for a bone density scan during or after cancer treatment — for example while on hormone therapy such as aromatase inhibitors — it is there to keep an eye on your bone strength, not to look for the cancer itself.
Is REMS, the radiation-free scan, a cancer test?
No. REMS is a bone-health tool, just like DEXA — not a cancer test. REMS (radiofrequency echographic multi-spectrometry) is a portable, radiation-free method that uses ultrasound radio-frequency signals to assess bone density, bone quality and fracture risk, reporting measures such as BMD, T-scores and a fragility score PMC11898608, 2025. Like DEXA, it is designed to answer questions about your bones — it is not designed to detect, confirm or rule out cancer.

A DEXA scan measures bone density to diagnose osteoporosis and assess fracture risk.
