What Is a Normal Bone Density Score by Age?
In the UK, a bone density T-score of -1.0 or above is normal, -1.0 to -2.5 is osteopenia and -2.5 or below is osteoporosis. Under-50s use a Z-score instead.
What is a normal bone density score?
A normal bone density score is a T-score of -1.0 or above. Below that, a T-score between -1.0 and -2.5 is classed as osteopenia (low bone mass), and a T-score of -2.5 or below is osteoporosis. These are the World Health Organization diagnostic thresholds, and they are used consistently across the UK and internationally StatPearls Publishing, 2024.
The Royal Osteoporosis Society frames the same UK ranges as normal (+1 to -1), osteopenia (-1 to -2.5) and osteoporosis (-2.5 and below) Royal Osteoporosis Society. The Bone Health & Osteoporosis Foundation sets out the identical cut-offs for postmenopausal women and men aged 50 and over Bone Health & Osteoporosis Foundation. A fragility fracture alongside a T-score of -2.5 or below is classed as severe (established) osteoporosis StatPearls Publishing, 2024.
What is the difference between a T-score and a Z-score?
A T-score compares your bone density to a healthy young adult at peak bone mass; a Z-score compares it to the average for people of your own age, sex and ethnicity. The T-score is the number used to diagnose osteopenia and osteoporosis, because it measures how far you have fallen from the strongest your skeleton is ever likely to be — a value of 0 means you match that young-adult peak NIAMS (NIH).

Is it normal for bone density to fall as you get older?
Yes — some decline is entirely normal. Peak bone mass is reached in your twenties, and roughly 95% of your adult bone mass is laid down by around four years after peak accretion in the late teens Osteoporosis International, 2016. After that peak, bone is gradually lost: areal bone mineral density begins a slow decline in both sexes from around age 40 and continues through adult life Therapeutic Advances in Musculoskeletal Disease, 2012.
For women, the pace changes sharply at menopause. The steep drop in oestrogen drives a phase of accelerated bone loss that can persist for up to around 10 years after menopause Therapeutic Advances in Musculoskeletal Disease, 2012. Low oestrogen after menopause is a major reason osteoporosis risk rises in women NIAMS (NIH).
Because the T-score always measures you against a young-adult peak, it is expected to drift lower with age — a gently falling T-score over the decades is the norm, not automatically a warning sign.
What is a good bone density score for my age at 50, 60 or 70?
There are no separate numeric T-score cut-offs for 50, 60 or 70 — the -1.0 and -2.5 thresholds are the same at every age. This surprises many people, but it follows directly from how the T-score is built: it compares you to a young adult, not to your peers, so the scale does not shift as you age. Any website offering an "average T-score for a 65-year-old" is misreading the measure.
To judge whether your bone density is normal for your age, clinicians turn to the age-matched Z-score. A Z-score around 0 is expected, and a Z-score of -2.0 or less means bone density is low, or below the expected range for your age NIAMS (NIH). In older adults, though, a low T-score is common and expected, so the Royal Osteoporosis Society stresses that the number is only part of the picture — full fracture-risk assessment matters alongside it Royal Osteoporosis Society.
Two more nuances worth knowing:
- Risk scales with the score. The chance of a broken bone rises roughly 1.5 to 2 times with each 1-point drop in T-score NIAMS (NIH).
- Density is not the whole story. Bone mineral density alone does not decide treatment — clinicians weigh it against your overall fracture risk Royal Osteoporosis Society.
Why do women and men have different bone density patterns?
Women reach a lower peak bone mass than men and lose bone faster, so age-related fractures tend to start earlier in women. Women have lower peak bone mass and smaller bones to begin with, and the menopausal oestrogen drop accelerates loss NIAMS (NIH). In men, bone mass typically peaks in their 20s at a higher level, and bone loss is slower — which is why the age-related rise in fractures starts later in men than in women NIAMS (NIH). Why does an early bone density baseline matter?
Because a greater peak bone mass built earlier in life lowers your risk of osteoporosis and fractures later on — optimising bone accrual during growth may be one of the most effective ways to prevent future fractures Osteoporosis International, 2016. You cannot rebuild your twenties, but you can find out where you stand today and track it from there.
