Losing Height and Stooping: Warning Signs of Spinal Fractures
Losing height or a new stoop can be a sign of spinal (vertebral) fractures from osteoporosis — and they're often silent. UK guidelines flag height loss of 4cm or more for a bone check. Here's what to do.
Can losing height be a sign of osteoporosis?
Yes — gradual height loss and a stooping, rounded upper back can be signs of spinal (vertebral) compression fractures caused by osteoporosis. The Royal Osteoporosis Society explains that spinal fractures can cause height loss and a curved spine or change in posture, and describes what actually happens: "A spinal fracture is when the bones in your spine squash down on themselves, rather than breaking apart." That is why the warning signs of osteoporosis in the spine look different from a typical broken bone. Instead of a sudden, obvious break, the vertebrae can slowly compress and wedge — and as several of them do, a person gradually loses height and the upper back curves forward. The NHS notes that some older people develop the characteristic stooped, bent-forward posture, and that "It happens when the bones in the spine have broken." NHS, 2022
It's worth being balanced here: a little height loss is a normal part of ageing, driven partly by changes in the discs between the vertebrae and by posture. But new or significant height loss, or a stoop that seems to be developing, is a signal worth taking seriously rather than shrugging off. How much height loss is a red flag?
In the UK, a history of losing 4cm or more in height is treated as a reason to check for spinal fractures. The National Osteoporosis Guideline Group states that vertebral fracture assessment (VFA) is indicated in postmenopausal women, and men aged 50 or over, if there is a history of 4cm or more of height loss or a kyphosis. Research points in the same direction. A 2020 study in Scientific Reports found significant, independent associations of historical height loss with prevalent vertebral fracture — an odds ratio of 1.89 (95% confidence interval 1.55–2.29) — with an optimal cut-off of about 4.95cm. So the two figures work together rather than clashing: think of 4cm as the UK clinical trigger for arranging spine imaging, and the ~5cm from the research as a broadly consistent, study-derived cut-off. The practical takeaway is simpler than the numbers — if you suspect you've lost several centimetres of height, that's worth mentioning to your GP.
Can spinal fractures happen without pain or a fall?
Yes — osteoporotic spinal fractures are often "silent," happening without an obvious fall and without the pain you'd expect from a broken bone. The Royal Osteoporosis Society is clear on this: "You may not always be aware a spinal fracture has occurred. Sometimes, spinal fractures don't cause the pain you'd expect from a broken bone." That silence is part of why bone loss can progress unnoticed. A 2025 update in SICOT-J reports that around two-thirds of these fractures are asymptomatic, meaning many go undiagnosed. Because a vertebra can compress without a dramatic injury, the outward changes — a gradual loss of height, or a developing curve in the upper back — are sometimes the first things anyone notices. That's exactly why paying attention to them matters.
What causes a stooped or hunched upper back?
A stooped, hunched upper back most often develops when several bones in the spine break and wedge forward from osteoporosis. The NHS explains that the characteristic stooped, bent-forward posture "happens when the bones in the spine have broken, making it difficult to support the weight of the body." NHS, 2022 The medical term for that forward curve of the upper spine is kyphosis, and the SICOT-J update notes that patients with vertebral fractures may present with kyphosis on examination. Alongside the visible curve, symptoms can include back pain. The NHS notes that broken bones in the spine are a common cause of long-term pain, NHS, 2022 and the International Osteoporosis Foundation observes that "Back pain and loss of height can be the first symptoms of vertebral fractures secondary to osteoporosis." It's important to keep perspective, though. A stoop isn't proof of osteoporosis on its own — posture habits and normal disc changes can round the upper back too. That's why the sensible move is assessment rather than assumption: only a proper check can tell whether fractures are behind the change.
What should you do if you notice height loss or a developing stoop?
See your GP — don't try to diagnose it yourself. If you've noticed new height loss or a developing stoop, your GP can assess your overall fracture risk and, where it's appropriate, arrange imaging to look for spinal fractures. The NOGG guidance describes vertebral fracture assessment being done with lateral thoracic and lumbar spine X-rays, or with lateral-spine DXA imaging — typically alongside a DXA bone density scan. The broader point is a proactive one. Because spinal fractures can be silent and osteoporosis often shows no symptoms until a bone breaks, waiting for pain isn't a reliable strategy. Establishing a baseline and monitoring your bone health early puts you ahead of the problem rather than behind it. Gradual height loss and a stooping upper back can be more than "just getting older" — they can be the visible signs of vertebral fractures from osteoporosis, which frequently happen without a fall or the pain you'd expect. UK guidance treats height loss of 4cm or more, or a developing kyphosis, as a reason to check for these fractures with a DXA scan and, where appropriate, spine imaging. Not every stoop or lost centimetre means osteoporosis — but new or significant change is a cue to get assessed, not to worry alone. The most useful thing you can do is move from noticing to knowing: mention it to your GP, and consider establishing a bone-health baseline you can monitor over time.
