Alcohol and Bone Health: Does Drinking Cause Osteoporosis?
Heavy or chronic drinking is an established, dose-related risk factor for osteoporosis and fracture. UK guidance is no more than 14 units a week — here's what alcohol does to your bones.
Does drinking alcohol cause osteoporosis?
Heavy or chronic drinking is an established, modifiable risk factor for osteoporosis and broken bones — but it is dose-related, so how much you drink matters far more than whether you drink at all. The NHS lists "heavy drinking and smoking" among the factors that increase the risk of osteoporosis and fracture (NHS, 2025), and the Royal Osteoporosis Society puts it plainly: "If you drink a lot of alcohol, your risk of osteoporosis and broken bones is significantly higher." (Royal Osteoporosis Society, 2024)
The scale of the effect comes from a large meta-analysis of 5,939 men and 11,032 women. It found the alcohol–fracture relationship is non-linear: there was no significant increase in risk at 2 units a day or less, but above that intake the risk climbed — by around 23% for any fracture, 38% for osteoporotic fracture and 68% for hip fracture — and, importantly, this was independent of both age and bone mineral density (Osteoporosis International, 2005). In other words, heavy drinking raises fracture risk through more than just thinner bones. How much alcohol is safe for your bones?
There is no separate "bone" limit — the simplest yardstick is the UK's general low-risk guideline of no more than 14 units a week. The UK Chief Medical Officers advise not drinking more than 14 units a week on a regular basis, ideally spread over three or more days with some alcohol-free days; one UK unit is 10ml (8g) of pure alcohol (NHS, 2025). The Royal Osteoporosis Society gives the same 14-units-a-week figure specifically in the context of bone health (Royal Osteoporosis Society, 2024).
For fracture risk specifically, clinical assessment draws the line a little differently. The FRAX fracture-risk tool treats 3 or more units of alcohol per day as a risk factor for fracture, and Osteoporosis Canada notes that three or more drinks a day increases osteoporosis risk (Osteoporosis Canada, 2023). That daily threshold and the weekly 14-unit guideline point in the same direction: keep it modest and spread out. How does alcohol weaken bones?
Heavy alcohol intake acts on the skeleton in several ways at once — it slows the cells that build bone and disrupts the hormones that regulate it. A review of alcohol's effects on the adult skeleton describes a biphasic, dose-dependent response: heavy intake decreases bone mineral density, impairs bone quality, and reduces bone formation by suppressing the osteoblasts — the cells responsible for laying down new bone (Alcohol Clin Exp Res, 2016).
Alongside that, heavy drinking alters the hormones and nutrients involved in bone and calcium regulation — including vitamin D, parathyroid hormone and testosterone (Alcohol Clin Exp Res, 2016). In men specifically, the NHS notes that alcohol misuse can lower testosterone, which is itself a risk factor for osteoporosis (NHS, 2025). Osteoporosis Canada adds that alcohol also contributes to poor nutrition, another indirect route to weaker bones (Osteoporosis Canada, 2023).
Does alcohol increase your risk of falls and fractures?
Yes — beyond what it does to bone itself, alcohol raises the chance of the fall that actually breaks a bone. The Royal Osteoporosis Society points out that as people become less steady on their feet with age, even mild intoxication increases the risk of a fracture from a trip or fall (Royal Osteoporosis Society, 2024). Osteoporosis Canada likewise notes that alcohol contributes to falls (Osteoporosis Canada, 2023).
This helps explain a striking detail from the meta-analysis: the rise in fracture risk at higher intakes was independent of bone density (Osteoporosis International, 2005). A weaker skeleton is only part of the story — impaired balance and reaction time do the rest, which is why fall risk matters as much as density when it comes to broken bones.
Does moderate drinking damage your bones?
The evidence for light-to-moderate drinking is genuinely less clear, and may be roughly neutral — this is not a reason to worry about the occasional drink within UK guidelines. The meta-analysis found no significant increase in fracture risk at 2 units a day or less (Osteoporosis International, 2005), and the skeletal review reports that light-to-moderate intake is generally described as neutral-to-beneficial, with some studies showing modestly higher bone density or slowed age-related bone loss (Alcohol Clin Exp Res, 2016).
Will cutting down on alcohol help my bones?
Yes — reducing a heavy intake appears to meaningfully lower fracture risk. In a study of 262 older men, researchers modelled what would happen if those drinking heavily cut below 3 units a day. Doing so would move up to 66.7% of the men who were at high risk of a major osteoporotic fracture, and up to 41.0% of those at high risk of hip fracture, below the high-risk threshold on the FRAX tool (Nutrients, 2021). Because alcohol is a modifiable risk factor, it is one of the levers you can actually pull.

Heavy or chronic drinking is a real, modifiable risk factor for osteoporosis and fracture. It slows the bone-building cells, disrupts the hormones and nutrients that keep bone healthy, and makes a fall more likely — and the meta-analysis shows the fracture risk climbs steeply above 2 units a day, independent of bone density (Osteoporosis International, 2005). Light-to-moderate drinking looks less clear and possibly neutral, so the practical takeaway is not to panic about the odd glass, but to keep within the UK's 14-units-a-week guideline and treat sustained heavy drinking as something worth addressing for your bones as well as the rest of your health.
